The registrant has permission to participate in all camp activities except as noted by me and/or an examining healthcare provider.

I will promptly pick up the participant if they develop an illness or fever 100 degrees. I hereby give permission to the physician selected by Peniel Bible Conference to order x-rays, routine tests,and treatment related to the health of the participant for both routine health care and in emergency situations. If I cannot be reached in an emergency, I hereby give permission to the physician selected by Peniel Bible Conference to hospitalize, secure proper treatment for, and order injection, anesthesia, or surgery for the participant.

I understand the information on the Health form will be shared as necessary with camp staff. I give permission to photocopy this form. Peniel Bible Conference has permission to obtain a copy of the participant’s health record from providers who treat the participant and these providers may talk with the camp’s staff about the participant’s health status. I agree to the release of any records necessary for treatment, referral, billing, or insurance purposes.

This child has my permission to fully participate at Peniel Bible Conference, 336Lake Tour Road,Lake Luzerne NY 12846, including planned off campus activities. I give my permission to Peniel Bible Conference to arrange necessary related transportation. I will be available by telephone if any situations arise that need my attention.

Peniel may use this child’s quotes, picture, and actions for multimedia publication (no names will be used).

Breach of Participant Code of Conduct Policy PBC reserves the right to send campers home for extreme behavior that violates the Code of Conduct. This will be at the family’s expense and without refund.
Cancellation and Credit Policy

If a registrant is a “no-show” for any program at PBC, the entire program fee is forfeit.

If there is need to cancel a summer program registration prior to May 1, Peniel will provide full credit of payments received that can be applied to another PBC program within one year. If cancellation is between May1-June 1, Peniel will provide a 50% credit of payments received that can be applied to another PBC program within one year. If cancellation is after June 1, no credit will be given.

If there is need to cancel a non-summer program registration up to two weeks prior to the event, Peniel will provide a program fee credit that can be applied to another PBC program within one year. If the cancellation is within two weeks of the event, no refund will be provided.

If cancellation is due to a participant medical or family emergency, PBC must receive prompt verbal notice of either of the above and the office must receive written medical verification by the participant’s physician. PBC will credit your program fee that can be applied to another PBC program within one year.

Peniel may need to cancel a program or event in the case of insufficient participants or extreme circumstances. Notification will be given as soon as possible and PBC will provide a payments received credit that can be applied to another PBC program within the calendar year.
